I am aware that mountaineering activities entail a residual risk of sustaining physical injury ranging from minor to very severe.

I acknowledge and accept the following:
- Sports and mountaineering activities in a mountain environment are subject to objective hazards—inherent to the activity itself—arising from environmental characteristics and weather conditions. The Mountain Guide will make every effort to effectively manage the risks associated with these hazards and has a duty to inform the participant about them.
- Participants must possess the appropriate level of physical and mental preparation for the activity they wish to join. To this end, they must inform the Mountain Guide of their technical proficiency, current physical condition, and any relevant health issues.
- The Mountain Guide reserves the right to cancel or modify the program—either partially or entirely—based on weather and environmental conditions, as well as the physical and/or mental condition of any participant.

I declare and confirm:
- That I have been fully informed about the activity in which I intend to participate, the characteristics and schedule of the program, as well as the necessary equipment, clothing, and specific technical skills.
- That I am capable of performing the aforementioned activity; that I am in good health and do not suffer from any medical conditions, injuries, or dependencies that could affect the performance of the activity.
- That I am aware that, although Mountain Guides make every reasonable effort to minimize risks, the nature of the activity involves dangers that cannot be entirely eliminated, and I consequently accept responsibility for them.
- That I will strictly adhere to the Mountain Guide's instructions and assume full responsibility for any actions taken independently—without following the Mountain Guide's instructions—in which case the Guide is authorized to verbally terminate the contract.

I authorize the processing of personal data in compliance with applicable personal data protection legislation, and in particular the European General Data Protection Regulation (GDPR) 2016/679 and Legislative Decree no. 196 of June 30, 2003 (as subsequently amended and supplemented, most recently by Legislative Decree no. 101 of August 10, 2018).
